Named Accountable GP for all Patients

All patients have a named GP who is responsible for patients' overall care at the practice.  If you wish to know who this is, please contact the practice.  The named accountable GP will take lead responsibility for the coordination of all services required under the contract and ensure they are delivered for their patients where required (based on the clinical judgment of the named accountable GP).  Although patients have a named accountable GP, they can see any GP in the practice.

Why have an NHS Health Check?

Why should I have an NHS Health Check?

Anyone can be at risk of developing heart disease, stroke, diabetes, kidney disease and some forms of dementia. These can be referred to as vascular disease. The good news is that these conditions can often be prevented, even if you have a history of them in your family.

Everyone between the ages of 40 and 74 (except people who have already been diagnosed with one of these conditions or have certain risk factors) will be invited to a free NHS Health Check every five years. You do not have to wait to be invited, if you qualify you can book your NHS Health Check Appointment at Reception now

Have your check and you will be able to take steps to maintain or improve your health.
