In line with the Department of Health’s guidelines, we recommend all patients with Asthma  to have an ‘15 minutes Asthma Review Appointment" with the Practice Nurse once a year in order we can ensure your treatment is suited to your condition.

In addition to this, please find below a Asthma Questionnaire which you can answer and submit to us but please note it does not replace the 15 minute Annual Asthma Review appointment in person with the Practice Nurse

If your symptoms are deteriorating or you have any concerns, please make an appointment to the respiratory nurse or a doctor as well.
